Abstract A lectin histochemical study was performed to investigate the glycoconjugate saccharidic moieties on the endometrial epithelium and stroma in 12 women undergoing controlled ovarian hyperstimulation COH for in-vitro fertilisation for embryo transfer IVF-ET in early luteal phase. Cytochemical controls were performed for specificity of lectin-sugar reaction. As far as the endometrial glands and stroma are concerned, the obtained data showed no differences in the endometrial lectin binding between the subjects of the control group and the ones undergoing COH, with the exception of PNA reactivity at the level of the apical portion of the glandular cells, which was detected only in COH women. It is noteworthy that, although the endometrial dating using the Noyes’s criteria showed marked dissynchronies between the stroma and the glands in COH subjects, a uniformity of lectin binding, revealing the same type and localization of terminal oligosaccharides, was observed in all the examined subjects. In all the treated subjects reactivity with sialidase-WGA and ConA, revealing the presence of Nacetyl -D-glucosamine and D-mannose respectively, was detected at the level of the lining epithelium.
Androgens, oestrogens and endometrium: a fine balance between perfection and pathology
Nanette Santoro, Laura T. To examine the relationship between endometrial histological maturation and reproductive hormones, we studied 11 fertile women, aged 18—37 yr. All participants had had at least 1 previous pregnancy and cycled regularly, every 25—35 days. Women collected daily, first morning voided urine for measurement of estradiol and progesterone metabolite excretion, estrone conjugates E1c , and pregnanediol glucuronide Pdg , respectively, throughout the cycle of study.
Hormones were normalized for creatinine. Between 7—9 days after home detection of a LH surge Sure Step , participants underwent an endometrial biopsy using a small bore Pipelle catheter.

Introduction: Leiomyomas are benign tumors composed of smooth muscle cells and varying amounts of fibrous connective tissue commonly encountered in women of reproductive age group. Leiomyomas need hormonal milieu for their growth and maintenance. Unopposed estrogenic stimulation manifests as leiomyomas undergoing secondary changes, endometrial proliferation or hyperplasia, and other associated pathological findings.
Objective: To study and analyze various histopathological changes within uterine leiomyomas in hysterectomy specimens. And also, to analyze the associated endometrial and adnexal structures pathology. Materials and Methods: A 4 years retrospective study from June to June conducted in the Department of Pathology and Obstetrics and Gynecology, ESIC Medical College and PGIMSR, wherein hysterectomy specimens clinically diagnosed as uterine leiomyomas were subjected to histopathological examination and relevant clinical data were analyzed.
Endometrial Cancer Screening and Diagnosis
The lining epithelium contains glycogen rich subnuclear vacuoles in the early secretary phase. The vacuoles will be supranuclear in midsecretary phase. In the late secretary phase, the glands are saw toothed and serrated with luminal secretions. This is called predecidual change as these are the precursors of decidual cells in pregnancy. The endometrium is obtained for evaluation by a process called dilatation and curettage or by endometrial biopsy.
This is done to evaluate infertility or dysmenorrhea.

Endometrial cancer is a cancer that arises from the endometrium the lining of the uterus or womb. The leading treatment option for endometrial cancer is abdominal hysterectomy the total removal by surgery of the uterus , together with removal of the Fallopian tubes and ovaries on both sides, called a bilateral salpingo-oophorectomy. Symptoms other than bleeding are not common. Other symptoms include thin white or clear vaginal discharge in postmenopausal women. More advanced disease shows more obvious symptoms or signs that can be detected on a physical examination.
The uterus may become enlarged or the cancer may spread, causing lower abdominal pain or pelvic cramping. Risk factors for endometrial cancer include obesity , diabetes mellitus , breast cancer , use of tamoxifen , never having had a child , late menopause, high levels of estrogen , and increasing age. Most of the risk factors for endometrial cancer involve high levels of estrogens. Higher levels of estrone in the blood causes less or no ovulation and exposes the endometrium to continuously high levels of estrogens.

Indian Journal of Pathology and Oncology. A two year histopathological study of endometrial biopsies in a teaching hospital in Northern India. Full Text PDF. Abnormal uterine bleeding AUB is defined as a pattern of bleeding that does not correspond with the duration, amount and frequency of the flow of a normal menstrual cycle. The causes of AUB vary with age; in young women in the reproductive age group, it is most commonly due to hormonal imbalance, while in peri-menopausal and post-menopausal women, AUB is generally due to hyperplasias and malignancies.
For endometrial dating, 4 μm sections stained with hematoxylin and expert pathologist according to the histopathological criteria of Noyes et.
Background: Abnormal uterine bleeding AUB is a common gynecological complaint associated with considerable morbidity and significantly affects the patient’s family, personal and social life. The aim of the study was to analyze the histomorphological patterns of endometrium in patients presenting with AUB and also to determine the incidence of AUB in various age groups. Materials and Methods: This is a prospective study, conducted in the Department of Pathology, in a tertiary care teaching hospital, Mangalore from October till date.
All cases of AUB with a probable endometrial cause were included in the study. Analysis was done in the form of percentages and proportions and represented as tables where necessary. Results: A total of cases were analyzed. Patients’ age ranged from years. AUB was most prevalent in the perimenopausal age group. Malignancy was detected in 6. Conclusions: Histopathological evaluation of endometrial samples is especially indicated in women over the age of 35 years to rule out malignancy and preneoplasia.
Among the patients with no organic pathology, normal physiological patterns with proliferative, secretory, and menstrual changes were observed. The most common endometrial pathology in this series was endometrial hyperplasia. Users Online:

A Papanicolau, G Lin. Intravascular Endometrium Mimicking Vascular Invasion. The Internet Journal of Pathology. Intravascular endometrium IEM is a rare finding that can pose a significant diagnostic dilemma, especially in cases of known carcinoma where the possibility of vascular invasion must be entertained. The distinction between IEM and intravascular invasion of malignancy can be made based on histologic findings and immunohistochemical profile.
We report a case of IEM in a hysterectomy specimen removed from a 38 year old patient with invasive cervical adenocarcinoma and adenomyosis. Histologically, the lumen and intima of large muscular vessels contained well-developed glands with high nuclear to cytoplasmic ratio and endometrial stroma with spindled nuclei associated with hemorrhage and hemosiderin-laden macrophages.
By immunohistochemistry, the benign glands within vascular spaces were positive for vimentin, ER and PR, but only rare cells were positive for p The stroma surrounding the intravascular glands was positive for CD These results confirmed the presence of IEM. The presence of IEM may be confused with vascular invasion which may affect staging and treatment of the patient. Endometrial tissue found within the myometrial vessels during menstruation is an uncommon, benign finding which was initially described by Sampson in We present a case of IEM, not associated with menstruation, in a hysterectomy specimen removed from a patient with invasive cervical adenocarcinoma.

Aetna considers the endometrial brush Tao brush an acceptable alternative to an endometrial suction curette e. Aetna considers sentinel lymph node mapping medically necessary for pathologic evaluation and surgical staging in persons with endometrial cancer. Aetna considers the following experimental and investigational because the effectiveness of these approaches has not been established:. The National Cancer Institute NCI, has stated that there is insufficient evidence to establish whether a decrease in mortality from endometrial cancer occurs with screening by endometrial sampling.
The NCI notes that based on solid evidence, endometrial biopsy sampling may result in discomfort, bleeding, infection, and in rare cases uterine perforation.

My approach to the interpretation of endometrial biopsies and curettings
A major proportion of the workload in many histopathology laboratories is accounted for by endometrial biopsies, either curettage specimens or outpatient biopsy specimens. The increasing use of pipelle and other methods of biopsy not necessitating general anaesthesia has resulted in greater numbers of specimens with scant tissue, resulting in problems in assessing adequacy and in interpreting artefactual changes, some of which appear more common with outpatient biopsies.
In this review, the criteria for adequacy and common artefacts in endometrial biopsies, as well as the interpretation of endometrial biopsies in general, are discussed, concentrating on areas that cause problems for pathologists.
of Pathology, in a tertiary care teaching hospital, Mangalore from October till date. All cases of AUB with a probable endometrial cause.
Five hundred endometrial specimens were studied to document the changes in blood vessels in various phases of menstrual cycle, menstrual disturbances and in unexplained infertility. Sixty-three cases were taken as control and cases as study group which included cases of dysfunctional uterine bleeding DUB , endometrial polyps, fibroids, adenomyosis, infertility and atrophic endometrium.
Using light microscopy, the vascular morphology was studied. The blood vessels were concentrated more in basal layer in the proliferative phase and in functional layer in the secretory phase. Cases of complex hyperplasia and pill endometrium had significantly higher vessel concentration. Congestion and dilatation of blood vessels were significantly higher in cases of DUB. The present study showed a positive correlation between endometrial angiogenesis and menstrual disorders. The alteration in blood vessel morphology has significant role in prognosis and in various anti-angiogenic therapies.
